From April this year we are changing the way we contact patients regarding their Long Term Condition reviews. Invitations to attend these annual reviews will now be sent in the month prior to the patient’s month of birth, with the aim of the review taking place in the patient’s month of birth, or as close to as possible.
Do we already hold your mobile telephone number and consent to us contacting you this way? If not and you’re happy for us to contact you via SMS messaging, please contact us to confirm. Providing us your mobile telephone number, means that any invitations sent, or confirmation of any appointments booked, will be delivered immediately to your device.
We shortly hope to announce the ability of booking these annual review appointments via our online services. If you have not already signed up to use our online services and would like to, please call the surgery for further details.
